[PT] Para além da influência genética, o ambiente familiar tem vindo a ser
demonstrado como um dos fatores mais decisivos, apesar de complexo,
no estado nutricional infantil. A obesidade parental está associada na
maioria dos casos a co-morbilidades, como a diabetes tipo II, doenças
cardiovasculares, hipertensão e dislipidémia. Pretende-se verificar a
existência de associação entre os antecedentes familiares da diabetes e
co-morbilidades e o estado nutricional das crianças portuguesas. Foram
avaliadas 5393 crianças com 6 (30%), 7 (44,6%) e 8 (25,4%) anos de idade
de 196 escolas do 1º ciclo do ensino básico. De acordo com os critérios
da OMS, 31,6% das crianças apresentaram excesso de peso, sendo
13,9% obesas. 13% das famílias das crianças indicaram ter diabetes tipo
II e mais de metade destas tinham-no associado à hipertensão arterial
ou hipercolesterolemia. Existe uma probabilidade 1,25 vezes maior de
crianças, de famílias com antecedentes de diabetes e colesterol elevado,
apresentarem excesso de peso, o que reforça a associação positiva entre
patologias familiares e o estado nutricional infantil. É fundamental uma
contínua avaliação detalhada e compreensiva das várias dimensões da
obesidade infantil, designadamente sobre o ambiente familiar, para a
adequada e mais ajustada resposta política e o desenho de programas de
prevenção e intervenção no combate deste problema de saúde pública.
[EN] The genetic influence, as well as the family environment, have been shown as one of the decisive factors, although complex, in the outcome of children's nutritional status. Parental obesity is associated, in most cases, with co-morbidities such as type II diabetes, cardiovascular disease, hypertension and dyslipidemia. Aims to verify the existence of association between family history of diabetes and its comorbidities and children´s nutritional status. A national representative sample of 5393 children, with 6 (30%), 7 (44.6%) and 8 (25.4%) years of age, was evaluated from 196 elementary schools. According to WHO criteria, 31.6% of children were overweight, and 13.9% obese. 13% of families in the study reported having type II diabetes and more than half of these had associated hypertension or hypercholesterolemia. There is a likelihood 1.25 times larger of children presenting overweight, whose families have a history of diabetes and highcholesterol, that reinforces the positive association between family pathologies and children´s nutritional status. It is essential a continuous detailed and comprehensive assessment of the various dimensions of childhood obesity. In particular, family environment should be one of the focus in order to facilitate adequate and more tailored policy response and the design of prevention and intervention programs in addressing this important public health issue.
