Participação em programas de avaliação externa da qualidade na fase pré-analítica – o papel do PNAEQ

Abstract(s)
A fase pré-analítica tem como objetivo obter amostras de qualidade e em segurança
para o técnico e o utente. Cabe ao laboratório implementar um sistema
de deteção, monitorização e redução ou eliminação de erros com impacto
nos resultados laboratoriais e na saúde do utente. Estas ferramentas são fornecidas
no programa de AEQ da fase pré-analítica do PNAEQ, cuja taxa de
participação é baixa. A realização de dois tipos de ensaios com intervenção
direta do PNAEQ foi utilizada como incentivo na participação no programa,
demonstrando que a avaliação de situações reais facilita a deteção de
ocorrências e a implementação de ações preventivas, corretivas ou de melhoria.
O cliente mistério simulou um utente com questões; na auditoria presencial
observou-se o procedimento de colheita e comparou-se a avaliação do
PNAEQ com a autoavaliação do laboratório. No primeiro, a taxa de participação
foi de 95% e a taxa de desempenho foi de 60%; no segundo, verificou-se
discordância de resultados em três pontos observados, para um dos laboratórios
auditados, e concordância em todos os itens para o outro laboratório
em observação. A integração destas ferramentas na rotina laboratorial permite
avaliar a competência dos colaboradores e verificar a adequação dos procedimentos
implementados no laboratório. A presença do PNAEQ nos laboratórios
promove o diálogo com os participantes e a partilha de orientações.
The aim of preanalytical phase is to ensure quality samples with safety procedures. The laboratory should implement a system for detection, monitoring and reduction or elimination of errors, which may have impact on the results and the health of the patient. PNAEQ preanalytical program provides these tools to laboratories. Due to the low participation rate, were launched two scheme types with PNAEQ direct intervention as an incentive to registration in the program, demonstrating that the evaluation of real situations facilitates the detection of occurrences and the implementation of preventive, corrective or improvement actions. Mystery Client simulated a patient with questions; in the presential audit, PNAEQ observed the collecting procedure and compared with the laboratory autoevaluation. In the first scheme, the participation rate was 95% and the performance rate was 60%; in the second, there was a results disagreement in three points observed for one of the audited laboratories, and agreement in all items for the other laboratory under examination. The integration of these tools in the laboratory routine allows the evaluation of the collaborators competence and verify the acceptability of the implemented procedures. The presence of PNAEQ in the laboratories promotes the dialogue with the participants and the guidelines sharing.
