Joint Action CHRODIS (Work Package 5)JA-CHRODIS is a European collaboration (Jan 2014 – March 2017) that brings together over 60 associated and collaborating partners from e.g. national and regional departments of health and research institutions, from 26 European countries. These partners work together to identify, validate, exchange and disseminate good practice on chronic diseases across Europe and to facilitate its uptake across local, regional and national borders. The focus is health promotion and primary prevention as well as the care of patients with diabetes or with more than one chronic condition (multimorbidity).
